چکیده
High risk patients with perforated peptic ulcer, where both the surgeon and anaesthetist are hesitant to operate, were managed by combined conservative management bilateral percutaneous peritoneal drainage under local anaesthesia as a damage control approach; followed either definitive surgery or omental patch closure. Here we study outcome in terms of morbidity & mortality such patients. A prospective 60 presenting VSSIMSAR, Burla was done between Feb 2018 Jan 2020. Study population includes ulcer confirmed pneumo-peritoneum X-ray bilious tap; high peri-operative (PULP score 8-18). They anaesthesia. The improved general condition definitive/ closure, rates noted. After stratification according PULP score, total number high-riskcases >8) (male-39, female-21); Majority them elderly (90%). Most common complication ARDS (28.3%). Maximum (75%) approach without significant complications offered exploratory laparotomy needful. Localised abscess observed around 13.33% patients; which image guided aspiration antibiotic coverage. overall rate 11.67%. Combined control, can be resource limited centre; closure is associated reduced mortality.

متن کاملLaparoscopic repair for perforated peptic ulcers with U-CLIP®
BACKGROUND The literature established that, in patients without Boey's risk factors, laparoscopic repair of perforated peptic ulcers, compared to open repair, is associated to lower wound infection rate, less analgesic use, reduction in post operative pain, shorter hospital stay. Some of the main drawbacks are length of operative time and laparoscopic surgeon's experience in intracorporeal knot...

OBJECTIVE To analyze the epidemiological profile of patients with gastroduodenal ulcer perforation and verify if the presence of H. pylori in the peritoneal and intraluminal secretions of these patients can be assessed by rapid urease test. METHODS We conducted a prospective, descriptive, cross-sectional study with data from patients in a hospital at a regional level, in patients with peptic ...

HYPOTHESIS Since the early 1990s, the laparoscopic technique has been increasingly used for the treatment of perforated peptic ulcer. It is important to validate a risk scoring system that can stratify patients into various risk groups before comparing the treatment outcome of laparoscopic repair against that of conventional open surgery. The scoring system should be able to predict the likelih...
